About the role
A long-serving hospice providing palliative and end of life care for people living in London, and rated Outstanding for their service, is hiring for two Care Coordinators to join the End of Life Care Coordination Service. This will be a 3-month contract with the possibility of extension and will be based in their service near Clapham Common. The working hours are 37.5 hours per week, worked in 12 hour shifts from 8am to 8pm throughout the week.

As the Care Coordinator, you will be working as part of a team under the direction of the Lead Coordinator. You will liaise with end of life care patients, their families and healthcare professionals with the key aim of providing coordination of and improving access to services provided by other health, social and voluntary care agencies.

Your role plays a vital part in supporting patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital. The nature of the role may involve empathetic interactions with anxious patients and their carers and families.

You will have the following skills and experience:

* Experience in a similar or related role
* Experience using variety of strategies to meet the needs of an individual
* Effective communication skills face to face, in writing and over the telephone
* Ability to communicate with sincerity and empathy yet show personal resilience
* Strong organisational skills
* Strong attention to detail & accuracy
* In posession of or willing to have an Enhanced DBS check
