Head of System Transfer of Care
£70,248, grade 16, 37 hours, permanent
Location: Flemming Court, Whistler Drive, Castleford WF10 5HW. This position will also be required to undertake home working.

We are looking to recruit to the Head of System Transfer of Care role working across the Wakefield Health and Care system, linking in with Kirklees as part of the Mid Yorkshire Hospital Trust footprint. This permanent opportunity is a senior position which will have oversight of all pathways, data, and resources to ensure effective hospital flow and safe transfer of care from hospital across the Wakefield system.
The post requires you to be responsible for leading and providing expertise in the development and delivery of work streams which support system flow. The success of collaboration between partners often relies on the maturity of pre-existing relationships within a system. These relationships are strong in Wakefield, with the Integrated Care Partnership creating a supportive environment within which partner organisations can share responsibility for the way the Health and Social Care system performs.
You will work in partnership with all health and care partners in primary, community, acute, social care, and independent sector care settings, as well as with partners across the Mid Yorkshire Hospitals Trust footprint. With senior strategic oversight of the Integrated Transfer of Care Team, you will work to the Local Authority Executive Discharge Lead and the Deputy Chief Operating Officer at Mid Yorkshire Hospital Trust. A key feature of this role will be to provide shared leadership and expertise to manage system pressures.
The role requires you to be confident in holding system leaders to account in order to achieve our aims, including trouble shooting, informing, and updating the Integrated Discharge Executive Group on progress, and developing and implementing appropriate and effective escalations. You will also act as the winter planning/system pressures co-ordination lead for the Wakefield locality working with other equivalent roles and leads in Kirklees.
