Sanctuary Personnel, an award-winning and renowned Recruitment Agency is looking for an experienced and dedicated Band 6 Nurse Practice Facilitator to work based in Barnet.

The salary for this permanent Nurse Practice Facilitator job is £36,070 - £42,712 per annum.

Shift Pattern:

Full Time 37.5 week - to include evening, nights and weekend shifts

Duties:

Support with the education and development of our care team
Provide clinical and holistic care and support to children with life-limiting or life-threatening conditions and their families
Manage care planning, symptom control and end of life care
Supporting the care team with their education, development and IPC
Ensuring holistic care is family-centred, needs-led, evidence-based and meets all statutory requirements
Applying competent clinical decision making and practice
Assessing, planning, implementing and evaluating care plans to meet the children's needs
Undertaking end-of-life care and provide symptom management including assessment and planning
Delivering post-death care
Contributing to the caseload management including managing referrals, ongoing and annual assessment, care delivery and transition
Responsibility for delivering safe and appropriate care
Undertaking day to day management, supervision and appraisal of staff
Participating in the 24/7 out of hours clinical on call service for the locality as required
Ensuring all risks associated with care delivery are managed effectively with incidents or concerns being reported / followed up in line with our policy
Undertake appraisals of Band 5 Nurses and Specialist Carers
Actively contribute towards quality assurance and practice development activities e.g. audit, benchmarking, service review, reflective practice and action learning
Effectively work within the legal, ethical and professional frameworks applicable to children's palliative care
Share professional expertise with colleagues, providing support and training as applicable
Responsibility for maintaining and updating own professional practice
Acting as an ambassador for the charity, maintaining positive and effective communication with other agencies involved with families receiving our services
Any other reasonable management requests
Work within legal and charity guidelines, ensuring compliance with CQC and other statutory regulations
Work within Noah's Ark Children's Hospice administrative and IT systems and guidelines and be aware of, and comply with the relevant rules and policies
Undertake further duties that may arise as the work of Noah's Ark continues to develop

Requirements of the Nurse Practice Facilitator:

RN Child / RSCN
Active NMC Registration

Professional knowledge acquired through degree or diploma or other professional training
Significant post registration experience with demonstrated experience of delivering care to children and families in a health care setting
Experience of assessment, development, implementation, delivering and evaluation programmes of care across a range of environments
Evidence of working within a professional multi-disciplinary team
Experience of managing a small group of staff and resources on a day to day basis, including practice supervision, mentoring and appraisal
Experience of working in a palliative care service
Experience of working with learning and physically disabled children and young people/mental health needs
Experience of providing community care services
Experience of working with volunteers either personally or professionally
Knowledge of current legislation relating to our work
IT literate
Knowledgeable of children's palliative care
Leadership skills
Ability to undertake a wide range of clinical tasks
Take responsibility for own learning and have a willingness to grow and learn
Excellent communication skills

Able to plan and manage own time
Able and willing to work within a palliative care environment
Ability to work independently and within a team
Able to develop effective working relationships

Ability to provide on call clinical care i.e. able to respond out of hours
Willingness to provide flexibility to travel and work outside normal working hours where required
Familiarity with the geographical area the hospice covers
Full UK driving license with access to a car
Willing to undertake an enhanced Disclosure and Barring Service Application (DBS)
Be aware of the confidential nature of the work of Noah's Ark Children's Hospice
