A large healthcare organisation is looking to appoint a Nurse Assessor to assist in reviewing and validation of referrals that are received by the Continuing Healthcare Team. This is a hybrid role of working remotely with two days working onsite.

The post will be working in the East Midlands on an interim basis for 3 months initially with a potential extension in place.

Main Responsibilities:

  • Responsible for facilitating multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures;
  • Responsible for the management of cases as directed by Clinical Team Lead;
  • Ensure appropriate packages of care are funded in compliance with the National Framework for Continuing Healthcare and funded Nursing Care;
  • Review the CHC and FNC service and care commissioned regarding quality and appropriateness to client needs;
  • undertake investigations, as required, into an individual's eligibility for NHS Continuing Healthcare following an individual challenge or dispute;
  • Ensure accurate and open communication and co-ordination with a range of organisations and individuals.
A successful candidate will have:
  • Current NMC registration with active pin;
  • Previous experience within a similar role working within the NHS;
  • Experience of working effectively with clinicians, social care and other partner agencies
  • Must have evidence of post registration clinical experience, in a variety of acute/community settings;
  • Knowledge of safeguarding agenda and processes;
  • Experience and understanding of evaluating and measuring the performance of health services.
