JOB TITLE: Supportive Housing Specialist

EMPLOYMENT TYPE: Part-time

SCHEDULED HOURS: 30 hours ( Flexible, M-F hours between 8am-5pm)

PROGRAM: Center Street I Case Mgt

SITE LOCATION: Manchester, CT

OBJECTIVE OF POSITION:

To work within a multidisciplinary team to increase the independence and self-efficacy of individuals who are chronically homeless and have SMI and/or co-occurring disorders through outreach support and counseling, direct assistance and training in living skills, advocacy and linkage.

By:

  • Coordinating the care of individuals who are chronically homeless and dually diagnosed including psychiatric/substance abuse, medical, housing, entitlements, financial, legal, social, family, education, vocational, transportation and other activities of daily living.


DUTIES & RESPONSIBILITIES:

  • Provide targeted case management such as establishing rapport with clients, writing comprehensive treatment plans, provides counseling and support, providing direct skill teaching, advocacy and assistance in obtaining/maintaining benefits, budgeting, treatment, medical services, employment, transportation etc.

  • Maintain family and natural supports as well as other collateral contacts to ensure quality client- centered care with duties such as assessing and involving natural supports in the treatment process, networking with homeless service providers, liaison with landlords and provide overall case coordination.

  • Assists in understanding federal/state guidelines for rental subsidies, tenant rights responsibility as well as basic understanding of housing law as well as conduct outcome surveys as required

  • Maintain documentation in compliance with agency and program standards.

  • Participates in staff meetings, supervision, trainings and other meetings as assigned.

  • Additional duties as required.


QUALIFICATIONS:

Education: Bachelor's Degree in human services or equivalent education and experience

Experience: 2 years experience working with the dually diagnosed (mentally ill, substance abuse)

Licensure/Certification/Registration: Valid driver's license. CPR/First-Aid Certification within 6 months of hire desired.
