Job description
Cielo Talent is currently conducting an executive search for a Director of Patient Care Services - Home Care on behalf of our client, Health First. Health First is the only integrated delivery network (IDN) in Central Florida.
The Director of Patient Care Services - Home Care will oversee clinical operations across the system. Departments included are field operations, quality assurance, intake, clinical supervisors, and development of service lines. The Director is fully engaged in providing uncompromised safety, superior quality, memorable customer experiences, and financial stewardship.
PRIMARY ACCOUNTABILITIES
1. Lead daily clinical operations for the agency through Bundled Payment Care Initiative (BPCI) oversight, CMS regulations (both Florida and National data for 5-stars), drive H-CHAPs metrics, PDGM maximization and LUPA oversight for metric driven accuracy. Prevent fraud, waste, and abuse both strategically and financially while directing quality patient care and customer experience.
2. Demonstrate and promote strategic thinking and drive problem-solving by empowering and supporting team members to assume responsibility and accountability within their scope of practice.
3. Serve as a change agent, championing and motivating all staff in understanding the importance, necessity, and impact, as well as the procedures and processes for implementation and sustainability.
4. Oversee HF Home Care policy and procedures among vendors, e-prescribing, safety, and regulations as it relates to the broad scope of homecare services provided.
5. Maintain and disseminate best practice clinical information obtained from across Health First, governing entities and from other IDN directive and external resources. To be sure that appropriate staff are compliant with new procedures.
6. Provide performance tools and a system of accountability by proactively monitoring leading indicators for patient safety, performance outcomes, top decile benchmarks, and develop and execute a plan to ensure continuous improvement for patient safety outcomes and customer experience.
7. Provide input and recommendations regarding the development, expansion or addition of homecare services or programs, gaps in services, equipment, procedures, or practices that will build upon or improve operational or clinical efficiency, increase the quality of patient care, and reduce expenditure. Intake emphasis here Reduce expenses/ proper (OASIS) coding /quality.
8. Stay informed of national clinical trends, innovation, and advanced technologies. Analyze, develop, and recommend how these trends or advanced technology can be pursued and implemented to enhance services delivered and increase patient and family satisfaction.
9. Analyze system-wide financial impact when making decisions and recommendations regarding resource consumption and growth.
Qualifications Required:
· Education: Bachelor of Science
· Hold or able to attain State of Florida RN License
· Five (5) years clinical management or leadership role
Qualifications Preferred:
· Master of Science in a clinical concentration or intent to enroll or is enrolled
· Innate ability to build strategic relationships with other leaders and physicians
· Must have a knowledge of home care, as well as an ability to 'speak the language.'
· Candidates must understand the scope of managing a large very matrixed operation.
