Responsibilities

Responsible for the delivery of nursing services in Green House Homes. Direct the overall Infection Prevention and Control Program (IPCP) in its activities in accordance with state, federal and local regulations. Support DON in the supervision of the Nursing department in accordance with state, federal and local regulations. Coach, mentor, lead and hold nurses accountable to achieve established clinical and satisfaction outcomes. Create a real home environment for the elders/guests and empower self-managed work teams. Facilitate policies and procedures are followed according to John Knox Village of Florida, Inc. guidelines. Provide ongoing clinical oversite to nursing staff by collaborating with clinical support team. Perform medical record review of all new admissions. Ensure accurate and timely follow-up of Interim Medication Regime Review (IMRR)/Pharmacy Report by nursing staff. Review changes in condition and perform clinical assessments to minimize re-hospitalization of guests and elders. Maintain effective communication with the Medical Director and other providers and vendors. Monitor and assist with admission order entry, assessments, etc. Communicate effectively with family members and guests on customer service concerns. Collaborate with CST as needed. Ensure nurse call response time is within the required time frame. Participate in QAPI initiatives as Infection Preventionist. Investigate and proved resolution of guest and family concerns. Ensure Accident and Incident Reports are investigated and completed for guests, elders and staff. Participate in implementation of Restorative Nursing Program. Review discharge charts for accuracy and completion to report trends to QAPI. Provide oversight of all clinical care equipment and supplies. Perform all other job related duties as assigned to promote the goals and objectives of the organization.

Qualifications

* Bachelors Degree of Nursing preferred or pursuing from an Accredited College or University.
* Registered Nurse in the State of Florida.
* Experience with high-volume of Medicare A/Managed Care/Post-acute Care and Bundled Payment initiatives. Experience in acute in-patient rehab of a long-term acute-care hospital.
* Proven ability to positively engage employees, organize processes to achieve quality care and positive Federal/State Survey, Quality of Care, Customer Satisfaction and Associate Satisfaction outcomes.
* Proficient in the use of Microsoft Word, Excel, Point click care.

Interpersonal Skills

* Outstanding English communication skills: verbal, written and listening.
* Demonstrated ability to effectively communicate during any situation.
