PROGRAM: Unity House STATUS: Full Time

POSITION: Case Manager/Housing Specialist

REPORTS TO: Senior Case Manager

FUNCTION:

The Case Manager's primary responsibility is to provide case management services to individuals/families who are residing in the Volunteers of America's Housing. This position develops and monitors a service plan in conjunction with each client assigned in order to empower clients, increase their self-sufficiency and to reduce the likelihood of reoccurring homelessness. The Case Manager will be responsible to assist the individual/family with obtaining needed access to medical, social, educational and other identified services.

KNOW HOW:

Bachelor's degree in social services or equivalent. Knowledge of crisis intervention and familiarity with available community resources essential. Case Manager position requires a person with flexibility to work as needed to accommodate the schedules of residential homeless programs.

PRINCIPAL ACTIVITIES:

* To develop case plans for individuals on assigned caseload.
* Assist in housing placement for families and individual receiving emergency housing vouchers.
* To provide case management, assessment and referral services to clients (including chemical dependency, mental health, child care referrals and educational programs).
* Provide case management for families living in a hotel prior to residing in shelter.
* To provide crisis intervention for housing clients and assist case managers.
* To maintain statistical data on all clients on caseload through reporting.
* To monitor and enforce compliance of residents with Housing Stabilization programs guidelines.
* To participate in case conferencing and regular staff meetings with residential homeless team.
* Assist with special projects such as Quality Assurance and to assist with ensuring compliance with COA standards and CRR requirements.
* Assist with documentation control on each Housing client in accordance with HUD guidelines, as assigned.
* To assist with data collection/reporting requirements as assigned.
* Performance quality improvement (PQI) duties as assigned by supervision & PQI committee.
* Other duties assigned.

2. To decrease the likelihood of the families returning to homelessness.

3. To comply with specified regulations and requirements in the Supportive Housing Program grant and with general Volunteers of America policies and procedures.
