TITLE: CLINICAL DIETITIAN(50K +)
LOCATION: PORTAGE, MI

SUMMARY:
As a Clinical Dietitian/Diabetes Coordinator, you will plan, direct and provide nutritional assessment and care to patients/residents and provide community nutrition education pursuant to the facilities mission and philosophy.
This position also acts as the diabetes coordinator by planning, implementing, coordinating and evaluating the diabetes program and providing diabetes education.
RESPONSIBILITIES:
-Collaborate with physicians, nursing, pharmacy and other healthcare team members/departments to determine, coordinate, and provide medical nutrition therapy.
-Collaborate with nutrition services to provide nutritionally appropriate menus, food options, and services for patients and visitors.
-Develops and provides education, training, consulting, and counseling services throughout the organization and for the community on topics including eating disorders, weight loss, and dietary modifications for healthy lifestyles.
-Participate in all discharge planning meetings and patient/resident team conferences.
-Participate as Consulting Dietitian and fulfills all consulting contracts.
-Coordinate, implement, and evaluate all UPHS - Portage Health policies and procedures related to diets/menus, drug/nutrition interaction, and nutrition education.
EDUCATION/EXPERIENCE:
Bachelors Degree in Dietetics required.
2 years experience as Dietitian in health care preferred.
Appropriate state licensure or certification.
Certified Diabetes Educator Certification preferred or willing to work towards CDE.
Annual Influenza Vaccination Required.
ADDITIONAL INFO:
Facility is a 36 bed hospital with ancillary services including the areas only Open Bore MRI, 64-slice CT scanner, PACS, nuclear imaging, digital mammography and other radiological and laboratory services.
Other services include cardiopulmonary, certified sleep disorder center, regional dialysis unit, pharmacy, rehab, audiology, a 12/7 hospitalist program, home care and hospice, and a 60-bed senior living community.
The facility is offering a salary range between 50K-75K based on experience. Will offer a complete benefit package including relocation/sign-on bonus.
