Job description

The Admissions Nurse's primary task is to complete the admissions process for incoming hospice patients. They also educate potential patients, their family members and their caregivers on the benefits of hospice and help guide them at a very difficult time.

This position works Monday - Thursday from noon until 9:00 p.m.

The starting wage for the Admissions Nurse (RN) is generally between $28.75 - $33.13 per hour for 40 hours.

The service area for this position is all of Allegan and Ottawa Counties and significant parts of Kent and Muskegon Counties as well as the northern portion of Van Buren County.

Responsibilities of the Admissions Registered Nurse include:

  • Determining if the patient is hospice appropriate. This is done through a chart review and in-person assessment.
  • Addressing the immediate care needs of the patient/family and explain hospice services, insurance benefits, and complete the required documentation.
  • Communicating patient, family and caregiver immediate needs, and significant issues to the patient's care team within Hospice, and obtaining any needed orders from the provider.
  • Arranging supplies, equipment, medications and other available services to ensure a smooth transition into hospice care.
  • Thoroughly document the admission for proper hand-off to the care team.
Qualifications for the Admissions Nurse include:
  • Being a graduate of an accredited school of nursing and having current Registered Nurse (RN) licensure in the State of Michigan;
  • Sound clinical judgment and ability to work independently;
  • Knowledge of and/or experience with end of life care; and
  • The ability to communicate effectively both verbally and in writing through concise charting.
