Responsibilities (includes but are not limited to):
Manage Caseload
- Manage a Caseload of 110-145 Program Participants & respond to participant inquiries
- Maintain required contact with assigned participants, including telephone calls and quarterly home visits
- Maintain email communication with participants, as needed and resolve participant issues
Provide participants with practical skills training in areas such as:
- Locating community resources;
- Recruiting, selecting, hiring, training, and supervising employees
- Purchasing goods/services & managing a monthly budget to pay for their direct care workers
Assist participants to:
- Enroll in the Personal Preference Program & complete required enrollment paperwork
- Identify community resources, services and supports to meet their life goals;
- Monitor participant health and safety through monthly phone contact and quarterly home visits (as needed);
- Maintain progress notes in accordance with NASW standards & report and respond to participant complaints and grievances;
- Act as mandated reporter for allegations of abuse, neglect, and exploitation
Required Skills:
- Strong verbal and communications skills. Ability to maintain appropriate etiquette and customer support techniques, including use of program and population-specific language and terminology.
- Ability to identify, assess and respond to the unique needs of individuals with disabilities.
- Consultancy mindset; the ability to stimulate others to look at traditional health care service systems in new and different ways.
- Ability to understand, support and explain to others the core tenets of self-directed services.
- Ability to recognize and maintain the confidentiality of all materials in the work setting.
- Understanding of modern office methods and practices; efficient with computers and Microsoft Office Suite software such as MS Word and Outlook.
- Ability to establish and maintain positive working relationships with federal, state and county agencies and other community stakeholders.
- Ability to generate required service documentation in an accurate and timely manner.
- Ability to work independently, with minimal direct supervision.
Qualifications:
- HS Diploma or equivalent required, BA or BS degree in related field preferred.
- 2+ years of related work experience, 4+ preferred

- A valid driver's license, access to a motor vehicle, and active vehicle registration, inspection and insurance certification required
