Job description

We are seeking a Full Time Case Manager/Service Coordinator (SC). The Service Coordinator ensures that children and their families receive the support and service they need, as authorized under the NJ Department of Health Early Intervention Program. The SC assists families with infants/toddlers with special needs (birth to 3 years) by meeting in the home with the family and other practitioners to plan and ensure delivery of EI services.

The qualified candidate must have a Bachelor's degree or higher from an accredited institution in social work, public health, human services, health related field or education AND at least one year of post graduate documented paid, professional experience working with children (birth to 5 who have developmental delays) and their families in a counseling, supporting or case management role. If the candidate has a social work certification (CSW) or is a licensed social worker (LSW) the one-year experience is waived. Excellent communication skills-both written and oral; proficiency with Microsoft Office, interpersonal and problem solving skills and the ability to travel extensively on a daily basis for community based appointments in Mercer County, including Trenton.
