Wellness Director (Hourly)

Position Description: Primary responsibility is to oversee the status and health condition of all the residents, coordinate care with family, outside agencies, and residents' primary care doctor. You will assist residents in all homes to assess health related issues and delegate the caregivers and instruct them on their daily functions with residents.

Qualifications: Medical Assistant, Licensed Vocational Nurse or Registered Nurse.

Duties and Responsibilities:

• Superior knowledge of the New Haven mission, staff and operations including commitment to "person-directed" care.
• As the director, to assist in the preparation and final approval on all service plans and assessments for all residents. These are to be competed upon admission, quarterly and change in health condition.
• To act as a liaison for health related issues to volunteers, families or the general public.
• Attend and participate in monthly care conferences and staff meetings.
• To inventory medication carts weekly with the assistance of the Administrator and/or House Manager.
• To train and monitor all staff in the handling and dispersing of medications.
• To ensure that all MARs are accurate and kept up to date in BlueStep.
• Provide information as required for Quality Assurance measures.
• Coordinate delegation of caregivers with RN delegator.
• LOCA Assessments with the assistance and final review of the Administrator.

Operations:

• Participate in weekend management rotation.
• Ability to participate in Admissions and Marketing as needed.
• Maintain team focus and winning attitude with all staff.
