Overview:
The Care Coordination Program works with families to coordinate services to ensure the health and well-being of the child(ren) is achieved. The Outreach & Enrollment Coordinator is responsible for reaching out to internal and external resources to enroll new participants who are appropriate and eligible for Care Coordination. This person will also track and monitor assignment lists of enrolled participants to ensure services are being provided. This person may carry a small caseload of newly enrolled participants.

Hours: 35 hours, Non-Exempt

Major Duties:
Community Engagement
Collaborate with other GSS programs to identify and recruit appropriate participants: those with a chronic disease, mental illness and/or substance use disorder, to facilitate early access to treatment and social services
Effectively communicates and shares information to participants with appropriate consideration for language, literacy, and cultural preferences
Begins to create and develop a strength-based relationship with prospective clients
Pro-active and ongoing communication between referral source and participant until a Care Coordinator has been assigned.
Purposeful transition of the newly enrolled participant to Care Coordinator.
Facilitates communication of role responsibilities to ensure the needs of the participant are clearly and efficiently met.
Effectively interfaces and collaborates with outside partners to enrich ongoing collaboration as well as onboarding new participants.
Takes feedback to improve professional development
Always maintains appropriate and professional work demeanor.
Compliance, Documentation and Data Management
Enrolls a minimum of 8-10 new participants each month.
Verify participants are eligible, including having Medicaid Insurance
Documents enrollment and outreach encounters in GSI in a timely fashion
Obtains all required enrollment consents, and supporting documents for all new participants
Completes initial and ongoing assessments to determine the appropriate level of care management
Coordinates and schedules psychosocial assessments with the participant, the family, and the clinical consultant.
Timely submits all required documents to the compliance department
Creates an up to date tracking system to monitor enrolled participants with assigned care coordinators
When carrying a caseload:Exceeds all expectations required of assigned cases including needs assessment and individual care plans
Ensures all contact requirements are met, and ideally exceeded.
Keeps up to date records for all assigned cases
Qualifications:
Bachelor's Degree with a minimum of 1-year direct services in care management preferred; will consider an Associate's degree with at least 2 years' experience.
Bilingual - English/Spanish required
Willingness to travel between boroughs
Must have strong interpersonal, organizational, and administrative skills
Must be a critical thinker.
Must have a commitment to work from a strength based, trauma-informed & youth and family development perspective
