What You Should Know About the Community Care Liaison Role:

* This is a full time position.
* The schedule will include Monday - Friday with hours of 8:30am-5:00pm.
* You will be supporting our Butler - Warren territories with physician offices and long-term care facilities as your assigned accounts.
* We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed.

Key Responsibilities:

* Generates referrals and admissions using business development strategies, within assigned territory.
* Assist SWCR Community Care Liaisons by providing statistical information on specific accounts or geographic regions.
* Utilize various data sources to craft value added messaging and scripting for referral sources based on statistical fact
* Propose conclusions and assist all liaisons and team leaders in developing value-added messaging for marketing plans as needed.
* Collaborate with team leaders to provide training to new liaisons in using Datalab and Netsmart as well as Playmaker.
* Develops and implements agreed upon marketing plan which will meet the goal of expanding the customer base in the market areas.
* Monitors and recognizes changing referral patterns in assigned area modifies strategies, to meet goals.
* Documents accurately all account contacts daily, meeting any identified account visit goals
* Responsible for meeting monthly and annual referral and admission goals in a specific territory.
* Identifies, develops and maintains key relationships in the territory, by participating in various functions sponsored in the medical community.
* Communicates complaints and grievances from patients and families, care facilities, and other caregivers to leadership for proper resolution in a timely manner.
* Promotes Ohio's Hospice through educational opportunities and community events in assigned territory.
* Assists all liaisons with transforming aggregating and analyzing marketing data from various sources.
* Participates in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
* Precepts new staff to support professional relationships with newly hired team members as required.

Qualifications:

* Bachelor's degree and /or active RN license in State of Ohio without board actions preferred
* Two years in a sales/marketing role within the healthcare industry with a proven record of accomplishments in direct sales and marketing.
* RN meets all qualifications of Ohio's Hospice.
* Computer skills sufficient to properly document services and communicate effectively, CRM experience preferred.
* Analytic Skills to work with data from multiple sources and ability to confirm validity of data.
* Determine conclusions and sort relevant information.
* Good communication skills and the ability to present findings, translate data into understandable formats for liaisons and other departments as needed.
* Ability to write and speak clearly and easily communicate complex ideas.
* Uses critical thinking to identify marketing trends.
* Ability to prioritize tasks and manage multiple projects.
* Must have attention to detail.
* Ability to drive during daytime, nighttime or inclement weather.
* Valid Driver's License with Safe Driving Record
* State Minimum Automobile Insurance Coverage
