This position is located in the Georgetown Healthsource Pediatrics & Family Practice offices.

Located at: 631 E. State Street Georgetown, OH 45121

Work schedule is between 7am and 5pm.

Required Degree: Masters degree in Social Work, Counseling, or Marriage and Family Therapy

Required License: LSW, LPC, MFT
Preferred Degree: LISW, LPCC, IMFT / CDCA

*SUD credential and experience preferred

We need highly motivated and independent individuals with good communication skills to work on a team that has regular collaboration with the doctors and nurses as well as other community service providers.

Our Integrated Health Therapist will:

Working closely with the doctors and nurses to provide an integrated services for the youth, families, and adults. You will complete diagnostic assessments, Individual Service Plans, and risk assessments.
