About the Job

We are looking to hire an Outpatient Case Manager for our Central Clinic. This position will provide outpatient recovery support services to clients suffering with severe and persistent mental illness; and perform daily tasks to assist clients with learning new skills of community reintegration, socialization and healthy lifestyles in a fairly independent work environment. Outpatient services staff will receive crisis de-escalation training, which requires performing various physical, hands-on maneuvers and techniques. Some travel within our community will be required.

This position works with a multi-disciplinary team in identifying client needs and determining appropriate community resources to resolve identified needs as well as assisting with coordination of transportation, health care arrangements and scheduling conflicts. Outpatient Case Managers provide skills training to promote successful community living for individuals suffering from mental illness who have high hospital utilization, minimal successful independent living experiences, lack of appropriate health care, cultural or language barriers, behavioral issues, and other characteristics that serve as barriers to independent living. Individual must be a team player and have the capability to organize work, set priorities, meet critical deadlines, and follow up assignments with a minimum of direction.

Work Schedule: Full Time (40 hours per week), 8 hour day shifts

Starting at $20.00+ per hour DOE/Credentials.

About the Program

Our Outpatient Program provides assessment and evaluation, treatment plan development, and provides clinically indicated outpatient therapy, case management, skills training, and psychiatric (medication) services. The Psychiatric Services Team operates as a branch within the Outpatient Program, specializing in psychiatric, nursing, and medication services. Psychiatric services are provided both on-site and remotely through telemedicine.

What You'll Need

1 out of 3 Criteria below:

  • Bachelor's degree in Behavioral Science: Psychology, Counseling, Social Work, or Human Development;
  • OR minimum three (3) years of verifiable mental health experience working with individuals living with a serious mental illness in a supervised social service setting;
  • OR equivalent combination of behavioral science education in addition to satisfactory supplemental mental health experience and training

Successful applicants MUST also have a valid Oregon driver's license, maintain a safe driving record, and the ability to pass a DHS criminal background check.
