About the Job
We have an exciting career opportunity for a Supportive Housing Therapist who possess a combination of skills and QMHP credentials to provide clinical services for our Southern Oregon supportive housing programs. This position requires the ability to perform comprehensive mental health assessments, treatment plans, case coordination with community providers and supervision for individuals with serious mental illness in a supportive housing setting. Additional duties include the training and clinical oversight of Supportive Housing Associates who provide the 24-hour supportive housing support to the residents. This position will work collaboratively with community agencies to ensure appropriate admission to the program and provide warm hand-off to behavioral health services aftercare. The SH Therapist will report to the Clinical Manager for clinical standards and quality improvement. Successful applicants must be accessible by telephone for emergency purposes and provide responsible support, documentation and follow-up in a timely manner.
Work Schedule: Full-Time (40 hours per week) 8 hour day shifts, with on call rotation.

About the Program
Our supportive housing programs include:
Swing Lane Supportive Housing is an exciting, newly constructed housing program that provides permanent, intensive Supportive Housing, as well as transitional Room & Board units for adults who have a serious mental illness.
Orchard Home is a non-licensed, furnished transitional housing program located in Medford, OR intended to serve up to 5 Jackson Care Connect Members who need extra supports for ongoing stabilization. Orchard is perfectly suited to be a step between Springbrook Crisis Respite Center and more permanent, independent housing. Clients of Orchard Home will have a maximum stay of up to 60 days, with case-by-case consideration of extensions where needed and appropriate. Staff will be on-site during the day, 7 days a week, to ensure client stability and to provide supports as needed.
East Grove Home is a non-licensed, furnished, Transitional Housing program located in Medford, OR serving up to 5 JCC Members who need extra supports for ongoing stabilization. This program is intended to be a step between higher levels of care and more permanent, independent housing environments. East Grove will have a maximum stay of up to 60 days, with case-by-case consideration of extensions where needed and appropriate.
What You'll Need
State of Oregon QMHP credentials:
- Master's degree in Psychology, Counseling, Social Work or related field; OR Bachelor's degree in Nursing and be Licensed by the State of Oregon
- MUST meet OAR requirement for QMHP by having completed TWO (2) YEARS of post-graduate clinical experience in a mental health treatment setting
Successful applicants MUST also have a valid Oregon driver's license, maintain a safe driving record, and the ability to pass a DHS criminal background check.
