JOB DESCRIPTION:
TO ASSURE QUALITY COORDINATION AND FACILITATION OF MULTIDISCIPLINARY NEEDS OF CHILDREN AND FAMILIES WITH VARIOUS AGENCIES AND SERVICES IN ORDER TO MEET THEIR MENTAL HEALTH NEEDS, AND TO ACT AS A FORUM IN BRINGING TOGETHER VARIOUS SOCIAL SERVICE DEPARTMENTS AND/OR SCHOOLS TO PROBLEM SOLVE AND COORDINATE ANY ARRAY OF NEEDED SERVICES. THIS INCLUDES ASSISTING AND EDUCATING FAMILIES WITH ELEMENTARY-AGED CHILDREN IN CUMBERLAND/PERRY COUNTIES BY PROVIDING INFORMATION, REFERRALS, ASSESSMENTS, AND LINKAGE TO ADDITIONAL SUPPORTIVE SERVICES TO EMPOWER FAMILIES FOR THE OPPORTUNITY FOR THE SUCCESSFUL EDUCATIONAL, EMOTIONAL, FAMILIAL, PHYSICAL AND SOCIAL FUNCTIONING OF THEIR CHILDREN WHO MAY BE AT RISK IN ONE OR MORE OF THESE AREAS.
ESSENTIAL FUNCTIONS OF JOB:
1. PARTNER WITH ELEMENTARY SCHOOLS IN CUMBERLAND AND PERRY COUNTIES TO MEETS NEEDS OF FAMILIES AND CHILDREN. PROVIDE CONSULTATION, EDUCATION, AND MENTAL HEALTH LIAISON SERVICES TO ELEMENTARY STUDENT ASSISTANCE PROGRAM WITHIN THE SCHOOLS REGARDING MOST APPROPRIATE MENTAL HEALTH SERVICES AS WELL AS COMMUNITY/NATURAL BASED SUPPORTS.
2. PARTNER WITH FAMILIES ON RESOURCES TO MEET FAMILY AND CHILD NEEDS REGARDING LIFE DOMAINS.
3. CONNECT FAMILIES AND CHILDREN TO NEEDED SERVICES AND SUPPORTS.
4. COLLECT AND INPUT DATA INTO HCSIS.
5. PARTICIPATION IN COMMUNITY INTEGRATION PROJECTS.
ADDITIONAL PRIMARY JOB DUTIES:
1. ATTENDS SCHOOL MEETINGS AND PROVIDES CONSULTATION TO FAMILIES AND VARIOUS PROVIDERS REGARDING COORDINATION OF SERVICES.
2. ACTS AS ADVOCATE FOR CHILDREN AND FAMILY ISSUES WITHIN THE COMMUNITY AND SCHOOL.
3. ASSISTS IN ASSESSMENT AND PRIORITIZATION OF PLANNING NEEDS AND HELPS FAMILIES AND SCHOOLS IDENTIFY NATURAL SUPPORTS WITHIN THEIR COMMUNITY.
4. MAINTAINS RECORDS AND PREPARES REQUIRED REPORTS.
5. TRAVELS PERIODICALLY TO PROVIDERS, CONSUMERS OR FOR MEETINGS AND OTHER JOB-RELATED DUTIES.
6. RESEARCHES AND EXPLORES COMMUNITY RESOURCES TO MEET FAMILYS NEEDS.
7. PROVIDES INPUT INTO EXPANSION OF COMMUNITY RESOURCES VIA INTERAGENCY COLLABORATION.
8. UPHOLDS NOTICE OF PRIVACY PRACTICES AND STANDARDS OF CLIENT CONFIDENTIALITY.
9. PROVIDE MAINTENANCE, POST-VENTION, AND TRAINING SUPPORT TO SCHOOL DISTRICT STAFF.
OTHER DUTIES OF JOB:
1. ATTENDS TRAINING SESSIONS, MEETINGS OR SEMINARS AS REQUIRED.
2. OPERATES COMPUTER AND SOFTWARE PROGRAMS AND OTHER OFFICE EQUIPMENT AS NECESSARY TO CARRY OUT JOB DUTIES.
3. Performs other job related services as required.
METS:
SIX MONTHS OF EXPERIENCE AS A HUMAN SERVICES CASE MANAGER; OR A BACHELOR'S DEGREE WITH A SOCIAL WELFARE MAJOR; OR A BACHELOR'S DEGREE WHICH INCLUDES OR IS SUPPLEMENTED BY 12 COLLEGE CREDITS IN SOCIOLOGY, SOCIAL WELFARE, PSYCHOLOGY, GERONTOLOGY, CRIMINAL JUSTICE, OR OTHER RELATED SOCIAL SCIENCES AND ONE YEAR OF PROFESSIONAL SOCIAL CASEWORK EXPERIENCE IN A PUBLIC OR PRIVATE SOCIAL SERVICES AGENCY; OR ANY EQUIVALENT COMBINATION OF EXPERIENCE AND TRAINING WHICH INCLUDES 12 COLLEGE CREDITS IN SOCIOLOGY, SOCIAL WELFARE, PSYCHOLOGY, GERONTOLOGY, CRIMINAL JUSTICE, OR OTHER RELATED SOCIAL SCIENCES.
SELECTION CRITERIA:
- MEET THE MINIMUM EXPERIENCE AND TRAINING FOR THE JOB.
- MERITORIOUS SERVICE: DEFINED AS THE ABSENCE OF ANY DISCIPLINE ABOVE THE LEVEL OF WRITTEN REPRIMAND DURING THE 12-MONTHS PRECEDING THE CLOSING DATE OF THE POSTING.
