Job description

Overview:
Provides assistance and oversight of the program, is the liaison between families and funding agencies, and assumes direct responsibility for the development, implementation, and monitoring of the program planning process. Acts as the Chairperson of the interdisciplinary team and ensures that all regulations/standards pertaining to the program management of individuals are in compliance with all regulations/standards governing Woods Services. Provides care coordination for all assigned individuals and families. Responsible for the provision of care coordination activities consistent with the program philosophy. Will be expected to work varied hours and days to ensure availability of services across shifts and to increase availability for the provision of family education and support. Works collaboratively with external Supports Coordinators and Case Managers.

Responsibilities:
Chairs all interdisciplinary team meetings including Individual Service Plans, interagency reviews, and funding authorization reviews. Ensures these meetings are held in accordance with the regulation timeline.

Completes all necessary authorization paperwork and packets to ensure funding. Provides assistance to families and individuals with Social Security issues, funding eligibility, insurance/benefit issues, and the guardianship process as needed.

Works with managed care organizations to ensure funding for residential services, one-to-one services (if applicable), insurance authorization, and completion of required paperwork to ensure residential funding for their assigned caseload.

Prepares individual treatment plans in accordance with Woods Services program planning guidelines and licensing and placing agency standards.

Ensures that invitations to service plan meetings are sent in a timely fashion.

Reviews all professional/consultant reports pertaining to individual's programs with the core team and is responsible for any follow up on recommendations.

Communicates with the core team on a routine basis. Facilitates continuity of client care ensuring inter and intra shift communications.

Responsible for working with the core team on the development, implementation, evaluation and monitoring of all individual's programs and general residence programs.

Ensures the implementation of restrictive procedures in conjunction with the clinical director and behavioral analyst staff. Ensure that restrictive procedures are monitored and documented appropriately.

Provides crisis management and facilitates emergency psychiatric hospitalization of individuals when necessary, under the supervision of the Clinical and Care Coordination Directors.

Completes monthly summaries of individuals progress.

Assists with the development and/or revision of the Comprehensive Biopsychosocial Psychiatric Evaluation, as needed.

Shares with families and funding agencies information on the individual's adjustment, changes in program, and other relevant information via telephone, letter, and on-site conference. Documents all contacts.

Participates in the orientation of new staff and the training and education of current staff through the provision of in-service training.

Coordinates discharge planning with families and appropriate agencies.

Responsible to work varied hours and days to ensure visibility across shifts and for evaluation of program activities.

Responsible for completing all required paperwork in a timely manner to ensure compliance with all regulations/standards governing Woods Services.

Serves as a role model to persons living in the program with respect to mature appropriate behavior: problem solving and coping responses, initiative, active listening and other social skills, punctuality, grooming and dress, and accepting direction from supervisory staff.

Participates in the development of client care policies and procedures.

Maintains professional and ethical relationships with families, legal guardians, and funding agencies.

Performs other duties as assigned.

Qualifications:
Required: Bachelor's degree in social work (or other related field) from an accredited college or university (Master's preferred).

Minimum two years experience in the social service field, and/or two years experience working with children and adolescents with autism or developmental disabilities.

Experience working with behavioral health insurance systems preferred.

Must have strong written and verbal communication, organizational, time management, multi-tasking and interpersonal skills.

Must be able to lift 50 pounds, pursue individuals on foot, physically restrain (with appropriate training), bend, squat, and reach.

Must be able to stand for several hours at a time.

Prior to employment, personnel requirements include three professional references, medical certification, and child abuse clearance.
