Job Summary Facilitates the flow of patients on inpatient units. Responsible for accurate patient placement and efficient patient flow, arranging for admissions for inter-facility transfers. This person will gather clinical information needed to determine where patients are to be placed, and work with Patient Placement RN and assigned unit to maintain effective patient flow. Communication is a key aspect of the position, including communication to physicians (bed availability, transfer issues, etc.), nursing units (admissions types, beds needed, etc.), co-workers, nursing supervisors, operations managers, and Bed Coordinators. Responsible for ensuring the very best patient/family experience by optimizing all customer interaction.
Job Duties

  • Gathers Clinical information on patient admissions.
  • Provides clinical screening on all admission requests with emphasis on urgent and emergent requests.
  • Screening is done for appropriateness and pre-certification.
  • Coordinates admissions with nursing units and requesting physicians, considering mode of travel and bed availability.
  • Ensures that bed assignment and placement occur in a timely manner.
  • Coordinates information flow between patient placement staff and pre-certification staff.
  • Assists patient placement with computer entry of admissions when necessary.
  • Collects data as requested Initiates collaboration with the physicians, ancillary personnel, and other interdisciplinary team members and takes action to proactively resolve and facilitate patient flow.
  • Suggests issues of improvement and offers solutions to the issues.
  • Supports and participates in initiatives that support the Magnet mission.

Position Details

*Hours: Day/Evenings with weekend rotation. No travel required.

*Inpatient experience is preferred.

Education Graduate from Specialty Training Program-Nursing (Required)
