Position Summary

A Caris Healthcare Chaplain provides support the hospice interdisciplinary team through office functions. The hallmarks of this position include completing comprehensive patient assessments, identifying care planning goals, and providing spiritual support to patients and caregivers at the end of life.

Responsibilities:
Completes and documents ongoing spiritual assessments, interventions, and outcomes related to patient and family

Communicates significant spiritual needs of the patient and family to the hospice interdisciplinary team related to terminal illness, impending loss, and bereavement.

Conducts visits with patients and caregivers in their residential setting.

Connects patients and caregivers with community clergy and/or spiritual leaders as needed to meet diverse spiritual needs

Assists with bereavement support and interventions for caregivers following the death of a patient

Participates in an on-call rotation to make after hours visits when needed for a patient's death, funeral, admission, spiritual crisis, etc.

Qualifications:
Master of Divinity Degree required

One unit of Clinical Pastoral Education is preferred

Prefer one (1) year experience in Hospice or related field

Excellent communication and interpersonal skills
