Job description

 

Required Skills and Experience:

•Empathy and a genuine interest in providing care for a geriatric population

•APRN or PA-C Texas license

•Strong interpersonal skills as you will interact with many team members at the company and partnering skilled nursing facilities

•Bilingual in English and Spanish preferred

•Post-acute care experience either in a nursing home or acute rehab setting.

•Specialty certification in Gerontology is a plus

Additional Job Details:

•Conduct in person visits with patients in skilled nursing facilities.

•Work with Physical Medicine and Rehab physicians to develop care plans and treatment recommendations for pain management and the improvement of functional and behavioral health in the geriatric populations.

•Attend weekly team meetings at skilled nursing facilities to help coordinate care and make treatment recommendations.

•Perform joint injections and botulinum toxin injections for spasticity as needed.
