POSITION SUMMARY
Quality Assurance Coordinator, in conjunction with the Charge Nurse shall be responsible for the gathering, assimilation and reporting of facility data. Laboratory Coordinator, in conjunction with Dialysis Coordinator shall be responsible for processing patient monthly, bi-monthly, and PRN blood specimens per OSHA guidelines. QA/Laboratory Coordinator reports directly to the Administrative Assistant.

- · Types at a minimum of 60 words per minute.

- · Complete assignments in a timely manner.
- . Medical Background - Dialysis experience preferred (May consider DT )

DUTIES & RESPONSIBILITIES
1. Assist the Charge Nurse with data reporting to the Network.
- a. Fistula First Report
- b. Par Report
- c. EM System Report
2. Update and maintain accurate records of the following:
- a. Machine Operation Log
- b. Venous Pressure Log
- c. Vascular Access Graph
- d. Vascular Access Utilization
- e. Patient Discharge Log
- f. Network 14 Event Report
3. Be in attendance of weekly nursing audit with the Charge Nurse, Clinical Nurse Manager Assistant and a billing department representative every Monday after 12:00pm to ensure the integrity of documentation you are responsible for.
4. Submit the following CQI reports by the 2nd of each month:
- a. Cumulative Laboratory Report
- b. Infection Trend Report
- c. Hepatitis Status Report
- d. Venous Pressure Log Summary
5. Prepare, Maintain, and Breakdown charts for the following patients:
- a. Hemodialysis Patients
- b. Peritoneal Dialysis Patients
- c. Harris County Sheriff's Office Patients
- d. Dallas County Sheriff's Office Patients
7. Request and submit medical records as needed.
- a. New Admissions
- b. Hospital Discharges
- c. Delinquent Information
- d. Facility Transfer
8. Ensure physician and patient satisfaction through quarterly surveys.
9. Maintain patient paperwork through electronic and physical filing daily.
10. Process all specimens, including mail outs, in compliance with OSHA guidelines. (Protective eyewear, gloves, gown, etc.)
11. Ensure blood labels, for specimens drawn by field nurses in remote areas are mailed out by the 20th of each month.
12. Ensure that appropriate requisition request and numbers are entered into the data base.
13. Per training, key in all patient laboratory results into the cumulative lab report monthly.
14. Distribute patient laboratory results to the Charge Nurse, Renal Dietitian and individual patient physician officedaily.
15. Ensure that monthly lab results are place on the Patient Care Conference reports and cumulative lab reports are place in patient folder for physician conferences.
16. Distribute patient laboratory results to the Charge Nurse, Renal Dietitian and individual patient physician officedaily.
17. Defrost and clean refrigerators in the laboratory monthly. (Keep an accessible & visible log)

Pay: $18.00 - $20.00 per hour
