Job description
The Supportive Services for Veteran Families (SSVF) Veteran Housing Specialist will work to ensure that any veteran experiencing homelessness in Salt Lake County is engaged and has access to housing resources. The goal of the SSVF program is to provide low-income veterans experiencing homelessness or at-risk of experiencing homelessness with a range of supportive services designed to promote housing stability.
This passionate self-starter will work for an agency that is both the states largest nonprofit housing provider and the states largest homeless resource center provider. The Veteran Housing Specialist will focus on street outreach, community partner outreach, and resource center outreach on a regular and frequent basis.

JOB DUTIES
* Identify low-income veterans and screen them to determine program eligibility for SSVF assistance.
* Conduct and complete initial eligibility screenings/needs assessments, perform paper and digital intakes in ClientTrack, create eligibility files with required documentation for all new clients.
* Collaborate with program supervisor in ensuring program fidelity, documentation accuracy, and intra-agency collaborations.
* Provide direct outreach focused on helping the veteran exit literal or chronic homelessness directly into permanent housing, following a Housing First approach.
* Responsible for helping the veteran create housing stability plans to include the provision of housing placement services, temporary financial assistance, supportive services, and community referrals as needed.
* Conduct follow-up calls/visits with SSVF approved veterans to ensure they are supported in reaching their housing stability goals.
* Engage and build rapport with the target population, other community outreach programs, local social services and public benefit agencies, shelters, faith-based and community-based organizations, and local VA facilities.

Requirements
EDUCATION AND EXPERIENCE
* Experience working with diverse or vulnerable populations required.
* Bachelors degree or equivalent experience required.
* Veteran candidates are encouraged to apply.

12. Demonstrates awareness of and sensitivity to diverse populations. Ability to contribute to the Agency's commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages Excellent organizational skills and strong computer skills.
13. Must pass a pre-employment background check and drug screen.
PHYSICAL AND EQUIPMENT REQUIREMENTS
Ability to lift 25 pounds.
Ability to sit, stand and walk for at least an hour at a time.
Must be licensed and able to drive an agency vehicle.
