The Patient Care Coordinator I/ Scheduler is a member of a dynamic team focused on coordinating all aspects of patient care. The role provides continuity of care by ensuring a smooth transition between care settings and by developing a trusted relationship with patients to identify and assist with each patient's individual needs that may arise related to the care they receive.
Responsibilities:
Triage calls from referring physicians, patients, family members, and others wishing to access care at SCCA.
Requests medical records from referring physicians and outside institutions, reviews them for content and completeness, and organizes them into designated categories for clinician review.
Schedules and coordinates patients' appointments.
Collects and enters patient demographic and insurance information and enter into registration databases.
Assists with the financial clearance process as necessary.
Receives and responds to patient feedback.
Responds to other patient requests as appropriate.
Distributes work to the Program Assistant II and/or Patient Care Coordinator Assistant as appropriate.
Qualifications:
REQUIRED:
High school graudate or equivalent
Ability to work independently, organizing, and prioritizing concurrent responsibilities and demands with commitment to detail
Ability to communicate verbally in person, on the phone, and in writing in a clear and concise manner
Demonstrated customer service skills with ability to empathize with patients and their families and have difficult conversations regarding scheduling and financial matters.
Proficient in the use of word processing (MS Word), email and data entry in a Windows-based environment
Ability to solve complex problems and meet patient needs
Ability to work in a high pressure, deadline oriented and complex health care environment
Ability to review work to identify and eliminate errors and omissions
PREFERRED:
Bachelor's degree (BA or BS)
Minimum of 2 years of experience in a patient care setting preferred
Experience with Epic or other scheduling and registration software
Knowledge of CPT, ICD-10 and medical terminology
Knowledge of related State and Federal insurance programs.
SCCA has a mandatory COVID-19 vaccination policy, and there are no exceptions for any employee who is patient-facing and/or requires access to SCCA facilities.
Exceptions exist only for employees whose positions are fully remote, with no required access to campus. As a condition of employment, newly hired employees requiring access to campus must provide proof of vaccination before their first day of employment.
