Era Living is seeking Escort Case Management Assistants to become the newest members of our Resident and Family Supportive Programs team in Seattle, WA. Flexible schedules available - work up to 40 hours per week depending on your preference and availability.
Primary Responsibilities:
This role supports the Resident and Family Supportive Programs by providing case management functions and other non-clinical services to residents. In this role, you are required to be available to respond within the scope of the program, on a pre-scheduled on-call basis for scheduled and/or emergency/unscheduled escorts.
- Escorting residents to healthcare provider offices, emergency departments or other healthcare settings; providing support for virtual/phone healthcare provider appointments
- Accompanying residents to other non-medical and non-financial appointments e.g. hair appointments, shopping trips etc.
- Fulfilling the requirements of an escort for example:
- Arranging/confirming transportation
- Communicating expressed/documented information provided by the resident, resident representative, and/or Community Care Team
- Communicating with resident representative, community staff, healthcare provider staff, and/or other Case Management Assistants to coordinate support
Compensation:
- $17/hr. for scheduled work performed (some travel time may be included)
- $27/hr. for unscheduled, emergency response during a scheduled shift (travel time included)
- $2/hour bonus pay for time scheduled on call but not worked
- Mileage reimbursement at current federal rate

Knowledge, Skills, and Abilities:
- Ability to work effectively as part of a multidisciplinary team
- Ability to multi-task and attend to detail
- Competent interpersonal skills that facilitate communication and interaction with residents, family members, and/or other third parties
- Ability to travel to multiple Era Living communities to perform duties as required
- Ability to respond and travel urgently during on-call shift to meet resident at healthcare provider location
- Ability to use computers, Word, Excel, Outlook and the Internet including smartphone applications
- Ability to foster a positive environment and represent Era Living in a proficient and professional manner; maintain professional boundaries and use discretion in consideration of stated visit
- Must be available to work at least 4 shifts per month
- Ability to read, write and communicate in English
- Ability to pass DSHS background check, required
- Must comply with the Washington State COVID-19 vaccination requirement for healthcare workers
Education and Experience:
- High School or GED, required; Bachelor's Degree in human services, social work, gerontology etc., preferred
- Experience working with seniors, preferred
- First Aid and CPR certified or able to obtain within 30 days of hire
- Complete Caregiver Dementia and Caregiver Mental Health training within 120 days of hire
