A Liaison within the community assisting residents with proper placement, transitions and health care needs. Providing
nursing recommendations as needed and being a referral resource to residents within Independent Living. Additional
responsibilities include the clinical assessment of leads/referrals for admission to ensure the referral meets community
clinical admission criteria and facilitate optimal transition of care.

Essential Job Responsibilities:

  • Check in with Independent Living resident or responsible party when a resident has a health crisis or is admitted to the hospital
  • Coordinate temporary care services in the Health Center or Assisted Living for Independent Living residents
  • Assist with coordination of emergency services when an Independent Living resident falls or calls 911
  • Help answer residents questions related to general health concerns
  • Provide house calls to residents in Independent Living if needed to check vital signs and make recommendations for care
  • Provide referrals to Home Health, Home Care, Therapy, Hospice or Health Care Providers in the community
  • Track residents health changes within the Independent Living and make recommendations to Executive Director when a transition to a higher level of care is warranted
  • Meet with potential residents and families and conducts assessments to determine appropriateness for admission.
  • Communicate with other department managers as necessary in relation to the assessment process
  • Supplement and support the Assisted Living Sales Counselor/Skilled Nursing Admissions Coordinator as needed, by responding to phone inquiries, setting appointments with qualified leads, giving tours and closing sales. Provides occasional skilled nursing admissions coverage as needed on the weekend or as part of a weekend rotation.
  • Participate regularly in sales meetings, training and seminars and participates in local associations and groups with memberships and agendas that impact area seniors as directed by the Health Services Director.
  • Partner closely with the Nursing Department to develop and manage clinical admission criteria and liaison information as necessary between the Community and the referral source while providing optimal response time to referral sources to achieve customer satisfaction.
  • Make evening or weekend appointments if necessary for a pre-planned appointment.
  • Participate in new hire orientation, providing training on blood borne pathogens or other topics as requested.
  • Maintain TB testing records and work with department managers to ensure compliance with new hire and annual testing requirements.
  • Coordinate podiatry and other Independent Living health services offered through outside providers.
  • Participate on staff and resident committees, including Resident Care Committee and Resident Healthcare Committee.
  • Available to travel as needed.
  • Other duties as assigned by Supervisor.


Education, experience, knowledge and specialized training requirements:

  • RN or BSN certification preferred
  • Experience in senior care/geriatric medicine and a sincere heart for serving seniors
  • Experience working within a multi-level retirement community setting preferred.
  • Demonstrated ability to effectively interact with people of diverse cultural, disability and ethnic backgrounds
  • Multi-faceted view of wellness including physical, mental, emotional and spiritual well-being
  • Exceptional customer service skills and focus
  • Excellent oral and written communication skills
  • Detail oriented and excellent organizational and prioritization skills
