Overview:

The Memory Care Activities Coordinator is responsible for planning, scheduling and leading meaningful activities in our Memory Care neighborhood and for leading activities in Assisted Living areas as needed. The activities should be appropriate to the needs and interests of residents and conducive to their physical, mental and social well-being.

Responsibilities:

· Assists in planning, scheduling and leading Life Enrichment programs that provide physical, mental and social opportunities for the residents. In addition, escort residents to activities, walk with residents in the hallways and bring residents to the courtyard when weather is appropriate.

· Memory Care will be the primary focus, with assistance in Assisted Living areas as needed.

· Makes daily, accurate chart notes on residents' participation in activities.

· Trains and coaches the nursing aides on conducting Life Enrichment programs with residents.

· Accompanies residents on outings on the bus. Assist the driver with helping residents on and off the bus and accompanying residents on the outing.

· Maintains an adequate supply of Memory Care activities including books, puzzles, crafts, games, etc.

· Meets with new residents to introduce and encourage them to participate in the Life Enrichment program.

· Helps plan appropriate programs for holidays and special events.

· Interacts with residents, guests and staff in an atmosphere of hospitality. Provides excellent customer service.

· Communicates resident information to the nursing department regarding activity ability changes that may require resident Care Plan updates.

· Protects the privacy of the resident when communicating oral and written resident information.

· Performs duties in accordance with published schedules.

· Stays current with all changes regarding residents and department operations. Checks schedules and activities to assist residents as necessary.

· Attends monthly meetings and stays current on in-service training.

· Promotes the Mission, Vision, and Values of Transforming Age in all areas of responsibility.

What we're looking for from you:

· Must be able to understand and follow written and spoken direction, and maintain accurate records.

· Must communicate well with others, observes confidentiality, and demonstrates a 'we care' philosophy.

· Must possess patience and sensitivity to others needs.

· High school diploma or equivalent in education, training and experience.

· One year/activities/life enrichment experience working with seniors.

· Memory Care experience preferred.
