Principle Responsibilities:
- Ensures daily observation and assessment of each resident. Directs and participates in resident care. Responsible for ensuring all aspects of the care plan are properly implemented
- Maintains open communications daily with the Director of Nursing Services on the status of all residents and staff
- Serves as a role model and provides leadership in problem solving among unit staff. Creates and promotes an atmosphere of respect, personal integrity and team work
- Leads the unit staff by ensuring proper orientation and training, and counseling/discipline on performance concerns
- Communicates with families and insurance companies to coordinate as much care and therapy as will be allowed by the residents' family
- Responsible for assisting the DNS in training, assists in performance evaluations, corrective and disciplinary actions when necessary
- Attends care planning conferences
- Ensures follow-through on dietary and pharmacy reviews, incident reports
- Works alongside team members to promote a team effort and maintain the highest quality resident service
- Assists DNS with overall leadership, direction and supervision to teams on the floor(s)
- Assists the DNS in the initial admission process to evaluate clinical and insurance information of potential residents
- Coordinates medical providers' appointments and writes complete discharge orders
- Actively supports the entire nursing staff where assistance is needed
- Works in conjunction with the Director of Nursing Services in completing the unit licensed staff annual performance evaluations. Works in conjunction with the unit nurse in completing the CNA reviews
- Integrates and supports the assessment, planning, and implementation of the admission, transfer and discharge process
- Ensures facility policies and procedures are followed
- Assures quality of care through completion of unit reports and assessments in care related areas such as infections, psychotropics (including AIMS reports), skin issues, weight reports and behavior issues
- Manages and coordinates the required resident assessment data collection process
- Assesses the status of the MDS forms for all current residents/patients. Establishes a schedule of known assessments to be done during the year with the Director of Nursing Services
- Starts and completes nursing section of MDS assessment. Ensures assessments are completed by other disciplines in a timely manner. Transmits completed MDS required materials to the State via computer
- Maintains knowledge of applicable laws, policies, and procedures regarding RAI/MDS
- Sets up monthly schedule for MDS assessments and distributes to staff and departments involved. Coordinates Medicare MDS assessments
- Researches and fills out MDS information for each resident due that month. Checks resident records and updates information as needed
- Consults with residents, families, and staff. Communicates resident information clearly and effectively to staff and management. Interviews residents and families as needed or required for information needed on MDS assessment and to help establish plan of care
- Participates in the admission, Medicare, and quarterly care conferences as needed
- Reviews, logs and completes the resident incident reports and provides copy to the Director of Nursing
- Coordinates follow through of the medical records process
- Monitors the utility and medication room for orderliness
- Promotes effective communication between the nursing staff, interdisciplinary staff, residents and families that demonstrates respect and cooperation
- Maintains a unit environment of safe work practices as a guide to quality resident care
- Participates in continuing education programs to maintain current license and upgrades nursing skills and knowledge of current trends and standards in LTC
- Assists in the organization of company-sponsored events, activities, and functions.
- Performs special projects as assigned by the DNS.
- Promote the Mission, Vision, and Values of Parkshore in all areas of responsibility;
- Other duties as assigned or may be necessary in an organization having overlapping roles and responsibilities.
- Coordinates and insures completion of resident restorative program for assigned unit.
- Helps to share clinical information with resident and family in care conferences.
Required Skills/Knowledge/Abilities:
- Knowledge of State and Federal regulations pertaining to long term care
- Knowledge and experience in IV therapy, starting peripheral IV lines, administering IV medication and performing blood draws for lab work
- MDS process and experience with Medicare requirements
- Demonstrated strong leadership and management skills; ability to direct and motivate the workforce
- Demonstrated competence in the nursing skills and procedures required on the nursing unit
- Exhibits strong assessment skills
- Demonstrated accurate utilization of the nursing process and prioritization
- Demonstrated excellent verbal and written communication skills; ability to explain policies and procedures and motivate staff compliance; ability to effectively communicate with team members, residents, family members and outside parties
- Ability to speak clearly and make self understood in a one-on-one basis with facility residents and in a variety of formal and informal group settings
- Ability to maintain strict confidentiality with sensitive medical information and foster a safe, ethical work environment
- Ability to delegate and assign duties to staffing as necessary
- Demonstrated excellent interpersonal skills and ability to maintain effective relationships with residents, families, staff, and members of the public
- Demonstrated excellent organizational, problem solving and conflict resolution skills
- Demonstrated proficiency with various software (Microsoft Outlook and Microsoft Word)
- Valid Washington State driver's license at time of hire in order to operate vehicles
- Good attendance is an essential requirement of this position
Education and/Equivalent Experience:
- Currently licensed in the State of Washington as a RN, with license in good standing
- Two or more years experience in a long term care setting as a licensed nurse
- Supervisory experience preferred
