*Hours: *40hrs per week

*Pay rates: *£35,692.80 pa

We would like to hear from experienced registered nurses who are committed, reliable, have high standards of care and have a keen interest in Care in the home and Clinical Complex Care.

The role will involve working in several Clinical Care and care in the home packages in the North West area, and will include (but is not limited to)

* Review and monitor care packages on a planned basis as agreed with the Manager/Clinical Lead.
* Respond in emergencies where packages of care are in danger of breaking down.
* Advise Case Managers, where necessary, regarding Clinical guidance and interventions.
* Attend appropriate training.
* Establish and maintain good liaison links with other healthcare professionals and attend MDT meetings.
* Assist client/carer by providing information on services and help to access them.
* Conducting all necessary correspondence and updating records
* Provide training in clinical areas for support staff and assessing competencies
* Undertake competencies and observations in clinical areas for support staff

* To carry out, out of hour's duties as and when required.
* To sensitively participate where appropriate with investigations into alleged misconduct or poor performance of support staff and or incidents arising from the provision of care.
* To carry out direct observations and spot checks on support staff. Implement any actions arising from these checks; ensure these checks are recorded

*Person Specification*

· Qualified Nurse with current PIN

· An understanding of the Health/Social Care sector

· Experience in Complex Care

· Ability to assess the needs of client's physical, psychological, and social needs.

· Excellent communicator with experience of working with people face to face and over the phone.

· Able to work under pressure

· Team Player

· Approachable
