Carer Support Wiltshire provides support directly to unpaid carers of all ages, as well as raising their profile and needs with health and social care professionals, employers, and the public.

An exciting new project in our Acute and Local community hospitals is being set up which will proactively identify and support carers in hospitals and those recently discharged, providing advocacy, assessments, support, signposting and referral.

The new Hospital Liaison Project involves working with health and social care professionals to identify and support carers who are, or who's cared for are in hospitals across Wiltshire including Royal United Hospital Bath and Great Western Hospital Swindon.

Key duties and responsibilities will include making initial contact with carers by phone and/or email to undertake a short light touch assessment then to signpost, advise or refer as appropriate based on assessment.

You will develop strong working relationships with key partners and individuals within the Flowhub and hospitals to ensure constructive partnership working and minimise duplication of effort. As a member of a small new team, the post holder will identify appropriate sources of information, advice, benefits, equipment, and services which carers can access and actively contribute and participate in publicity events and represent Carer Support Wiltshire and to raise awareness of carers' needs.

The people we are looking for will have a good standard of education, GCSE or equivalent and an understanding of carers' issues and rights. You will have experience of working in a health or social care environment and able to work flexibly within a team.

Hours of work: Full Time

Contract type: Temporary (12 months with a possibility of extension)
