Job description

We are delighted to be working closely with one of the leading and most luxurious care providers in the UK. I'm looking to appoint a Dementia Care Manager for their luxury nursing home in Wimbledon, London.

Our client is looking for qualified candidates who will be responsible for the leadership and management of the dementia community, acting as the dementia advisor for nursing and residential communities.

My client currently operates in excess of 20 care homes, providing leading services in residential care and the development of specialist services for residents with higher dependency needs. This includes; nursing care for infirm elderly, respite care, step-up and step-down care, end-of-life care and an award-winning specialist dementia service that is studied internationally.

Key Responsibilities

- Management of, and responsibility for, the overall operation of the dementia community in accordance with CQC/CIW regulations, evidence- based practice in dementia care, and Hallmark policies and procedures, from initial assessment onwards
- Promotion of high standards of relationship centred dementia care at all times
- Acting as an ambassador for the Hallmark Together dementia strategy within the home, developing the service in line with the strategy objectives
- Working alongside nursing and care teams to ensure Individual care plans are person centred and of high quality, and that all documentation meets Hallmark & regulatory standards.
- Taking ownership of team levels and ensuring the resources are in place to deliver quality dementia care and meet residents' needs.
- Liaising and building relationships with external professionals
- Championing relatives' involvement in all aspects of care and care planning, including life story work, John's Campaign, and provision of family meetings
- Working in partnership with the home's lifestyle lead to ensure that an appropriate programme of activities, and opportunities for meaningful occupation are in place
- Liaising with the head of relationship centred care through regular meetings to ensure on-going understanding of key objectives relating to the Hallmark dementia strategy
- Working in partnership with the regional dementia relationship practitioner to develop high quality relationship centred dementia care
- Planning in regular opportunities to work alongside the care team on shifts to provide support and coaching (day and night)
- Ensuring that all dementia community team members complete mandatory training within specified.
- Managing the performance of team members within the dementia community and ensuring regular reviews and appraisals take place.
- Taking ownership of and leading on induction training for new team members, including Hallmark's specific dementia care induction.

Experience Required

- Has achieved, or is willing to work towards qualifications NVQ 4 or 5 / QCF 5 in Health and Social care
- A minimum level 3 qualification in dementia care or experience & Knowledge of evidence- based practice in dementia care with at least 4 years' experience in long term care of people living with dementia.

- A detailed knowledge of care plans and advanced care planning.
- Evidence of supervisory and management experience.
- Academic qualification in Management and Leadership (or willing to work towards),and/or commensurate experience
- Thorough working knowledge of safeguarding legislation, DoLS, and management of referrals.
- Thorough working knowledge of the Mental Capacity Act, and experience of carrying out mental capacity assessments.
- Thorough working knowledge of care standards aligned to National Regulation and experience of the CQC registration & inspection process (CIW and RISCA regulations in Wales if appropriate)
- Knowledge of health and safety procedures, including legislation
- Theoretical knowledge and practical experience of implementing psychosocial interventions for people living with dementia
- Theoretical knowledge and practical experience of supporting people living with dementia who express needs through behaviour
- Understanding of budgeting processes within a care home setting
- Evidence of CPD in relation to dementia care
