Summary
Job Purpose
• The Young People's Support Worker plays a key role in the delivery of personalised care and support to vulnerable children and young people with a wide range of care and support needs which are delivered within group settings in a variety of community, building based and residential settings.
• The post holder will be required to actively support people using the service, in either 1:1 or 2:1 to achieve planned, personal objectives which look to maximise independence, promoting health, and wellbeing and community involvement
• The post holder will champion excellent care and support, promoting a customer focused ethos at all times. Ensuring the well-being of people using service the post holder will lead the service in the delivery of, innovative person centred enablement activities which promote independence, wellbeing, choice maximising community engagement.
• The service will be delivered from a number of community locations, and the post holder will normally be lone working with the young person they're supporting. However, a manager will be available by telephone to provide advice and guidance when required.
• The service is delivered 7 days a week and includes the delivery of services up to 10.30pm and the post holder must be prepared to work flexibly outside of normal working hours, including bank holidays and weekends.
• Whilst contributing to the delivery, continuous improvement and development of service, the post holder will support the promotion and marketing of the My Options Services as a leading provider of quality care and support services within Telford and Wrekin.

Major Tasks
Specific tasks:
• Supporting the Resource Co-ordinator & Activity Leader, the post holder will deliver a range of high quality activities that are accessed by young people with learning and physical disabilities, which will include behaviours that challenge.
• To adhere to all policies and procedures required to deliver the required service and meet regulatory requirements.
• Promote and maintain a customer focused ethos at all times, ensuring the well-being of customers and the delivery of person centred care and support, including all aspects of personal care. Promoting independence wellbeing choice and control.
• To contribute creative ideas to support the development of the customers.
• To be responsible for the safety and welfare of the children and young people accessing the service.
• To be responsive to an individual's needs as detailed within the framework of their personal care and enablement plan, working flexibly to respond to any changing needs
• To ensure the wellbeing of those people using the My Options Service by providing agreed support with all aspects of personal care, health and wellbeing support as detailed in the customers personal plan
• To actively support customers both one to one and in groups to achieve agreed objectives, gain new skills and encourage new experiences. Promoting confidence and community engagement
• To communicate effectively using verbal and non-verbal skills
• Record information to required standards, reporting all incidents in line with service and council's policies and procedures e.g. health and safety
• Attend and contribute to team meetings as required
• Actively work with the Resource Co-ordinator & Activity Leader to introduce new work practices to ensure operational effectiveness, financial efficiency and generate growth.
• Ensure all necessary policies and procedures relating to the service are understood and adhered to.
• Ensure that Health and Safety legislative requirements are complied with during the provision of support and activities e.g. risk assessments, food hygiene, fire records, COSHH etc.
• Ensure excellent working relationships with internal and external partners and wider organisations. Promote Telford and Wrekin's My Options services and activities to all stakeholders.
• Support a culture of continuous improvement, engaging with My Options management and staff to ensure good practice is shared and adopted.
• Have a flexible approach to work commitments in order to meet the changing needs of the service
• Carry out any other duties commensurate with the grade.
Generic tasks:
• Contribute to, and deliver the Council's vision and priorities both internally and externally.
• Ensure that people accessing the service receive high quality outcome focused personalised support that meets their needs.
• Through the delivery of agreed support plans, support the service to meet its objectives and targets within agreed timeframes.
• Support the Resource Co-ordinator & Activity Leader to ensure services are delivered efficiently and effectively minimising waste, taking action to ensure expenditure remains within budget and compliance with the Council's procurement policies.
• Ensure that Health and Safety legislative requirements are complied with during the provision of support and activities e.g. risk assessments, food hygiene, fire records, COSHH etc
