Description

To assess and support the housing needs of the Centers/Program chronically homeless and persistently mentally ill adult clients, to optimize their independent living skills for the purpose of securing permanent housing

ESSENTIAL DUTIES:

Implement housing procedures and standards of the Hamlin Recovery, Resilience, reintegration (RRR) Program.

Responsible for intake and assessment of potential clients to include: interview of client, accessing records, pre-application paperwork, and speaking with appropriate professionals and family support systems.

Assist clients in the application process for Section 8 assistance with all necessary paperwork and collection of documentation.

Assist clients in securing an apartment in the community to include transportation of client in apartment search, interface with potential landlords/property management companies, and maintain an accurate and up-to-date vacancy list in the surrounding community.

Facilitate the Section 8 101 group with qualified applicants.

Oversee the Center's transitional living property, Doris Foster. Act as a liaison between the Program and the Property Management Company, interview potential residents for Doris Foster, assist in their move in/move out, conduct bi weekly inspections of the units and facilitate a housing meeting, maintain spare keys to the property, report all maintenance concerns to the Centers maintenance department as well as to the Hamlin RRR Program Manager/Center wide Housing Coordinator/RRR Coordinator.

Maintain a caseload between 1-5 clients, assisting in the development of a comprehensive service plan, providing psycho education re: mental health symptoms, teaching independent living skills including, but not limited to: money management, budgeting, accessing community resources, monitoring activities of daily living (ADL'S).

Develop and maintain Homeless Section 8 and Continuum of Care (COC) files, submit monthly and quarterly reports to HACLA.

Attend all SA II housing meetings in the community.

Other duties as assigned by the Hamlin RRR Program Manager/Center wide Housing Coordinator/RRR Coordinator.

Attend housing referral meetings at other programs to review clients' readiness for level of housing.

Qualifications

Bachelor's degree in psychology, social work, or related field.

Two (2) years experience preferred in the mental health field working with chronically mentally ill adults. Experience in client advocacy preferred.

Must have a valid California Driver License in good standing and be insurable by the Center's auto insurance carrier.

Bilingual Spanish strongly preferred.
