Job description

Medix is looking for a self-motivated and passionate Registered Nurse OR Licensed Clinical Social Worker for our Full-Time Care Manager opening in the Washington DC area. This is an exciting and dynamic position, as the Care Manager, you will be responsible for ongoing assessment of health, environmental, and psycho-social status of members assigned to you, and ongoing care management of members through telephonic & field visits. ESSENTIAL FUNCTIONS, RESPONSIBILITIES AND DUTIES

Under the direction of the Senior Care Manager, the Care Manager (CM) is responsible for providing comprehensive care coordination services to members with high complexity medical and behavioral needs who are enrolled in the health plan. This CM will have a caseload of medically fragile members with a focus on medical intervention and associated care coordination needs outlined by the member assigned acuity level. The CM routinely communicates with members on their assigned caseload to assess their needs; develop, monitor and evaluate treatment plan progress; and assist with coordinating care and referring to appropriate services for members who have highly complex medical and psycho-social needs. Care Managers: 100% REMOTE because of COVID. Prior to COIVD: week would be set up: 1 mandatory office day, (admin day, team meeting day) rest of the week, set your week up the way you want (face to face with caseload, do notes, etc) VERY FLEXIBLE, Would have some in person meeting/training etcs. AFTER COVID (eventually) Care Manager will need to go back to in person meetings (home visits, doc appointments, public place).

3-5 Must Have Skills/Qualifications:

• LICSW or RN licensed
• Looking for at least 2 years of experience but open to new grads if need be (open to inpatient experience)
• For RN's, they must've at least applied for a DC license by the time they start
