The Pediatric Palliative Care Nurse LPN/RN provides nursing care to pediatric palliative care/hospice patients consistent with standards of practice, state, and federal regulations while following DPH/hospice protocols and procedures in the administration of care. LPN's and RN's must consistently demonstrate clinical proficiency, appropriate and timely documentation, and the ability to function as a successful member of an interdisciplinary team.


Responsibilities:


  • Interact with Managers and/or Directors on a professional level to ensure administrative needs are being met.
  • Utilizes, recommends, obtains and utilizes physicians orders.
  • Responsible for the planning, coordinating, providing and documenting care for terminally ill patients and their family members from day of admission through day of discharge.
  • Assists the RN Case Manager to develop, update, and implement a palliative care/hospice care plan that meets each patient's specific needs and is in compliance with federal and state regulations, reimbursement guidelines as well as the organization's policies and procedures.
  • Participates in the on-call rotation as assigned by the Clinical Director.


. Requirements:
  • Graduated from an accredited school of nursing for Licensed Practical Nurses
  • Maintains a current LPN/RN license in the state of Massachusetts
  • Experience as an LPN/RN commensurate with one of the following:
One year home health care experience within the last two years; orOne year community health or public health experience within the last two years;One year hospice experience within the last two years
  • Valid state driver's license and reliable automobile with proof of current automobile insurance
  • Demonstrated leadership capabilities with the ability to relate well to staff and other professionals
  • Must maintain a valid drivers license and dependable transportation
  • Expected to participate in appropriate continuing education as required to maintain nursing licensure
  • Possesses excellent verbal and written communication skills
