Job description
Our Health Center Locations: Brick, Freehold, Lakewood, Little Egg Harbor, Manahawkin, Manchester and Toms River; with our school-based Wellness Programs located within the Clifton Ave Grade School, Lakewood; and Lakewood High School.
OHI services include family and internal medicine, pediatrics, behavioral health, nutrition, OB/GYN women's health, dental, family planning, specialty care, pharmacies, PrEP, and a STI clinic.
Hours:
Full-time hours are M-F 8:00am- 4:30pm with rotating evening/Saturday shifts (subject to seasonal changes and business/site needs)
Position Summary:
Reporting to the Practice Director and the Practice Site Coordinator, the Patient Access Representative (PAR) is responsible for all patient registration, accurate documentation and data entry, discharge and financially related data collection. The PAR is a key contributor to the overall patient experience and will maintain a high level of customer service during the patients visit to the Health Center.
Responsibilities
Customer Service:
- Interacts with patients throughout their visit, leaving them with a positive experience
- Continuously demonstrates a professional demeanor when engaging patients and all Health Center visitors
- Engages all OHI staff and patients with a positive approach
- Works in an organized, efficient manner to keep a smooth and orderly flow of patients moving in and out of the medical office.
Registration and Documentation:
- Obtain consent for medical treatment, patients signature confirming/accepting receipt of patients' privacy notice, HIPPA rights, medication history, NJIIS consent, and financial responsibility.
- Screens all patients for the marketplace/NJ family care for presumptive eligibility.
- Completes a Household Assessment (HHA) on any uninsured/under insured patient.
- Ensures that all necessary fields are completed and that all required OHI/Letter of Agreement (LOA) documents are collected for all household members.
- Follow up via phone calls/mail, with incomplete HHA applicants to ensure all required documents are collected prior to the 2 nd visit.
- Collects and inputs all required HHA documents utilizing the print document barcode (barcode-sandwich) and label the document following the approved OHI abbreviation.
- Collects patient's co-pays as assigned by the completed HHA and sliding fee scale, and accurately applies the co-pay to the appropriate visit and provides patient with a receipt.
- Completes Advance Directives section as outlined in the policy
- Ensure all insurance information is captured correctly and update insurance information as needed.
- Verifies insurance primary care provider (PCP) is an OHI Provider and completes a PCP change form if this is not the case utilizing the Athena chart export.
- Obtains necessary patient release forms in accordance with patient confidentiality and medical records policy.
- Verifies all documents are accurately completed and signs full legible name when PAR signature is required.
- Completes payment plan form when needed for procedures.
- Accurately complete the required data entry for NJIIS.
- Collects and add the inbound transition documents as needed.
- Update the daily patient demographic form at every visit and obtain an updated demographic form from patient in an annual basis. Accurately update athenahealth to reflect changes.
Data Entry:
- Accurately completes all required data in athenahealth
- Accurately completes all required data in EHS
- Maintains a proper file system for proper organization of work.
- Collects and add the inbound transition documents as needed.
Financial Responsibility:
- Ensures daily cash and credit card collection is reconciled with athenahealth reconciliation report.
- Follows the end of day procedure policy to prepare daily deposits.
- Follows the cashbox policy to assure the Patient Access Representatives (PAR) assigned cashbox is reconciled daily.
Patient Engagement:
- Registers the patient for the EMR patient portal. Demonstrates portal functionality, provides education, illustrating the ease of communication between patient and care team and strongly encourages patient to access their protected health information (PHI).
- Participates in ongoing quality initiatives.
- Assists in the discharge process of patient, provides all necessary documents, and ensures documentation in the chart is updated with accurate information.
- At time of discharge, explains referral process and follow up care instructions. Completes all referrals needed immediately by the patient.
- Assists walk in patients to achieve a positive outcome.
- Is responsible for following the medical records procedure when records are requested.
- Sends newborn documents to Medicaid as needed.
- Is responsible for the scanning of all patient documents into their chart as needed
Competencies:
- Maintains an in-depth working knowledge of all technological operation systems including the EMR to ensure operational effectiveness.
- Participates in daily huddles with clinical teams and PSC, completes daily staff assignments and keeps log.
- Adheres to organization policies and procedures.
- Attends all mandatory in-services and meetings required by administration.
- Constantly strives to achieve excellence in day-to-day operations of respective department.
- Completes all mandatory in-services in a timely fashion.
- Adheres to Corporate Compliance Policies.
- Maintains a level of attendance as to meet the organization's needs as measured by total tardiness and call outs.
- Completes all reports as assigned by Practice Directors in a timely fashion.
- Reports any equipment problems through the IT request portal.
- Consistently demonstrates The Code.
- All other duties as assigned.
Requirements
Education and Experience:
- High School diploma or equivalent is a requirement.
- 1 to 3 years of experience in an applicable healthcare setting is preferred.
- Computer literate with the ability to learn customer service, and EMR applications.
- Duties require professional verbal and written communication skills, and the ability to type 30 wpm
