Job description


Full time Service Coordinator (SC) for our North Brunswick office, which is currently virtual with a planned return to office. This NJ Department of Health grant funded position serves families with infants/toddlers with special needs (birth to 3 years) living in central NJ. The service coordinator completes the initial phone contact with families using a web-based system. The service coordinator is part of team and must complete a minimum number of referrals on a daily/weekly basis with accuracy and speed. Additional responsibilities as assigned.



The qualified candidate must have a Bachelor's degree or higher from an accredited institution in social work, public health, human services, health related field or education AND at least one year of post graduate documented paid, professional experience working with children (birth to 5 who have developmental delays) and their families in a counseling, supporting or case management role. If the candidate has a social work certification (CSW) or is a licensed social worker (LSW) the one-year experience is waived. Candidate must also have excellent communication skills, proficiency with Microsoft Office, interpersonal and problem-solving skills. Supervisor skills a plus.
