Description/Qualifications:
Your Role With Us:
The Care Management Associate - Insurance Program Navigator is a bilingual in Spanish professional member of an interdisciplinary team working with assigned patients, completing tasks to ensure proper utilization of an insurance premium charity benefit.
Core Functions:
Process new submissions and ensure all necessary information is in place for committee review. If approved, connect participant with insurance broker at first available date during open enrollment. Ensure enrollment occurs.
Complete every year for currently enrolled participants with initial assessment completed prior to submission to committee for approval.
Educate participants about the need to apply for SH Charity Assistance every 6 months and OHP every year.
Navigator will meet with each participant in person on a yearly basis to assist them in signing the IAP Agreement.
Navigator will continue to assess engagement and interest in partnering to improve healthcare, meeting with participants as needed.
Track each participant's eligibility dates on a spreadsheet.
Act as primary point of contact for participant and Salem Health.
Educate participants about program details and ensure they understand their responsibilities in order to remain enrolled in the program.
Work independently, with supervision as needed, to support transition planning according to team identified timelines.
Background/Expertise:
Bachelor's Degree. Healthcare related field preferred.Consideration may be given to candidates who are willing obtain degree within 2 years, must sign an education agreement
One (1) year in a healthcare/community benefit setting using similar skills and performing similar activities preferred.
One (1) year medical setting using similar skills and performing similar activities preferred.
