Responsibilities:
The Home Care Coordinator is responsible for coordinating, scheduling, and delivering home care services to the residents of our Continuing Care Retirement Community.
Home Care Coordinator Essential Duties and Responsibilities include, but are not limited to:
Participates in hiring of new home health aides, and is responsible for orientation of new caregivers to the program.
Maintains current files for all employees within the Home Partners department.
Schedules shifts by matching caregiver qualifications and availability to resident's needs
Visits residents after referrals are made
Supervises caregivers and completes performance appraisals
Performs initial and ongoing evaluations, caregiver introductions and supervision of services
Completes appropriate visit records in a timely manner as per Wesley Enhanced Living policy
Takes calls after hours and weekends to handle emergencies and scheduling issues
Provides care to residents as assigned on the Plan of Care
Observe and report any changes in resident's mental, physical or emotional condition to the supervisor in a timely manner.
Complete required documentation in Care Binder of services delivered
Evaluates and supervises staff for conformity with professional standards of personal and companion care, and in accordance with state regulatory and home care licensure requirements
Enters, maintains and corrects resident billing and related accounting data
Participates in the quality assurance reviews and evaluations of Home Partners services
Qualifications:
Related Experience: 1-2 years of supervisory experience preferred; 1-2 years of Home Care experience preferred
Education or Degree Requirements: High School Diploma or GED required
Licenses, Certifications, Registrations: Certified Nursing Assistant certification preferred
