Job description

Overview:
The incumbent conducts screens for potential families. Completes Parent Surveys with families and makes appropriate referrals as necessary. Employees will manage Priority Reports and record appropriate data into the database. The employee is subject to call back and overtime as required by the hospital.

Responsibilities:
Establishes a trusting relationship with families by initiating and maintaining regular family contact.

Ensures compliance with grant regulations and requirements as directed through the Department of Health.

Conducts Parent Surveys with consenting families.

Consistently follows Healthy Start/Healthier Beginnings policies and procedures.

Has working knowledge of parent-child interactions, child development issues, and the dynamics of child abuse and neglect.

Consistently applies this information in working with the families.

Has familiarity with community resources and makes appropriate referrals to meet the needs of families.

May be responsible for community outreach for the area served.

Meets with supervisor on a weekly regularly scheduled basis for reflective supervision and to review assessments.

Documents Parent Survey information in accordance with Healthy Start/Healthier Beginnings policies and procedures.

Self-motivated to manage referral list and schedule of Parent Surveys.

Attends and participates in regular staff meetings, case conferences, in-service training, and other meetings as required or directed.

Can collaborate, with consent, with other organizations and agencies to provide a comprehensive array of services to families.

May participate in discharge planning for families, closely monitor outcomes of the participants who are still in the Neonatal Intensive Care Unit or on the Mother/Baby Unit as required or directed.

Acts as a mentor to new staff during the training process with the direction of the supervisor.

Demonstrates the ability to work cooperatively with co-workers as part of a team.

Demonstrates maturity in interpersonal staff relationships.

Qualifications:
EDUCATION:
Bachelor's degree in a related field from an accredited college or university required
Must complete all Healthy Families America training components
Must complete all AIMHI-TN components as required by the State.

LICENSURE, REGISTRATION, CERTIFICATION:
Valid Tennessee Driver's License required.

EXPERIENCE:
Two (2) years of experience working with or providing services to children and their families preferred.
