The MDS Coordinator leads by example, helps drive a positive culture, and is responsible for managing the resident assessment information of the Minimum Data Set (MDS) forms, ensuring that they are completed, reviewed for completeness and accuracy, and processed in a timely manner according to regulation, policies and procedures.
Principle Responsibilities:
· Manages and coordinates the required resident assessment data collection process.
· Assesses the status of the MDS forms for all current residents/patients. Establishes a schedule of known assessments to be done during the year with the Director of Nursing Services.
· Reviews results of last DSHS Survey to determine areas of needed improvement in the MDS process.
· Starts and completes nursing section of MDS assessment. Ensures assessments are completed by other disciplines in a timely manner. Transmits completed MDS required materials to the State via computer.
· Maintains knowledge of applicable laws, policies, and procedures regarding RAI/MDS.
· Ensures that supplies and forms are stocked and available.
· Sets up monthly schedule for MDS assessments and distributes to staff and departments involved. Coordinates Medicare MDS assessments.
· Researches and fills out MDS information for each resident due that month. Checks resident records and updates information as needed.
· Reviews and updates care plans as necessary.
· Consults with residents, families, and staff. Communicates resident information clearly and effectively to staff and management. Interviews residents and families as needed or required for information needed on MDS assessment and to help establish plan of care.
· Participates in the admission, Medicare, and quarterly care conferences as needed.
· Assists in resident care and evaluations as needed.
· Reports observations and concerns to the Director of Nursing.
· Establishes and maintains regulatory reference notebooks.
· Performs other duties as assigned.
Required Skills/Knowledge/Abilities:
· Willing to work as a member of a team in the Health Center and work under the direction of the Director of Nursing Excellent organizational & management skills.
· Must be able to read, write, and speak English.
· Must relate to residents and families with patience, caring, and understanding, and be detail-oriented, thorough, self-directed and accurate.
· Must have excellent communication skills to elicit and interpret patient information. Good computer skills required.
Education and/or Equivalent Experience:
· Four year college or university education.
· Current State Licensing as a Nurse with a minimum of two years of experience with long term care population.
