About the Job
We have an opening for the Veterans Case Manager position in the Southern Oregon Region that will handle Supportive Case Management. The successful applicant will become a member of a dedicated team whose mission is to work with homeless Veterans and their families to coordinate and monitor successful community living through various voucher systems or even fully independent housing. Specifically, this position helps connect Veterans with qualified housing, and helping them maintain that housing as they stabilize in other areas of their lives. This may include assisting them with obtaining/maximizing benefits, helping them gain independent living skills, interfacing with landlords, avoid eviction/address notices of correction, accessing public transit and services, and connecting them to services that help them maintain sobriety and improve their mental health. Case Managers are provided ongoing training and support, as well as opportunities for advancement. This is both an interesting and rewarding job working with a very deserving population. Work will generally occur at the Veterans Resource Center, various other ColumbiaCare locations and out in the community, including client homes. Frequent travel is required and safe driving is an essential part of this position.
Work Schedule: Full-Time (40 hours per week) 8 hour day shifts

Starting at $18.00+ an hour DOE/Credentials.
About the Program
Grant Per Diem Case Management works directly with Veterans by assisting them to transition into housing from GPD and HCHV programs, retain their housing once placed, and further develop the skills needed for successful independent living in the community. The Case manager provides follow up care, coordination and other supportive assistance to Veterans to help them maintain residence and learn the skills to become as stably housed and independent as possible in the community.
What You'll Need
- Two Years experience working with Veterans in supportive role
- Knowledge in the area of community resources and familiarity with VA vouchering and rental assistance systems (such as HUD VASH, SSVF, Rental Assistance Programs, etc.).
- Preference will be given to Veterans who can use personal experience to assist others toward independent living.
- Experience working with homeless individuals and/or the public housing authority.
- CPR certified or Basic Life Skills training course
- Ability to travel (mostly with-in the community)
